Replacement Passport Book
The U.S. State Department requires that the passport application be completed online, on their website. We have divided the process into 2 steps. Each link will open in a new window or tab. Please, close the new window or tab to return to this page.
The passport must have been issued within the last year to apply for a replacement passport. If the passport was issued over a year ago the applicant must apply for a Passport Renewal.
- Special Note - Do NOT include return shipping fees on checks for the US Passport Office. They should be included with payment to Perry International.
- Step 1, Our instruction packet explains the process to be followed after completing the online application in step 2. It contains our order form, a required authorization letter, and our instructions to assist in obtaining a US passport. You can complete the order form online before printing, or by hand afterwards. - Step 2, Complete the Department of State passport application online. The Contact Information page of the application should be completed using 79 W. Monroe St. # 1225, Chicago, IL 60603, in Care of Perry International. This link is to the US Department of State website. Passport Application
